MUSE School admission forms

This page links you to all the necessary forms for applying to MUSE School CA. Please let us know if you have any questions. We’re happy to help.

All applicants

Everyone who applies to MUSE School must fill out an application form. You can print the form and mail it, along with a check for your application fee. Or you can apply online. If you choose to apply online, please pay the application fee on line, as well.

Early Childhood Education (ECE) program applicants

• Childcare giver evaluation form

Elementary program applicants (grades K-5)

• Teacher evaluation form (K-5)
• Records request form (if student previously attended school)

Middle school program applicants (grades 6-8)

• Teacher evaluation form (6-8)
• Records request form (if student previously attended school)
